How to File a Claim and Form (Liability)

umbrella with word riskIf you become a part of an accident or involved in theft for which you believe the Fremont Unified School District has caused a loss to your person or property, you have the option of completing and filing a liability claim for damages form with the district. Each claim will be reviewed by the district's Risk Management department and a determination will be made to accept or reject your claim based on an investigation.

You will be notified of any decision regarding your claim.

Carefully read and complete the FUSD LIABILITY CLAIM FORM and return it to the Fremont Unified School District Risk Management Department at 4210 Technology Drive, Fremont, CA 94538. Please note that claim forms must be complete, signed and submitted within the appropriate timeframe to be eligible for processing.
